This form is used for Kingborough Lions United F.C. volunteers to submit their working with children registration receipt and copy of card for our records. If you have obtained the card for volunteering with KLUFC specifically, you can choose to ask for reimbursement. Once submitted, we will review details and if you have requested reimbursement we will arrange payment to the account you have provided.
